The problem with this solution is that the adapter is a Mini-Display port connection, while the card has 4 full sized Display Ports. Then rebooted, then opened up my display settings and clicked on 3840x2160 and viola, it works. I used SwitchResX and created a custom resolution, set 3840x2160 at 18 Hz using CVT-RB. Its a stock MBP with the Intel 4000 chip. I believe the problem is the cable, that it is not providing the DVI-Dual Link ability. I finally got this to work on my 2012 macbook pro retina (although I hear that non-retina versions work too).

Well if your mid 2012 MacBook Pro is a MacBook Pro with Retina display (15-inch only for mid 2012), then you dont need to do anything, as the MacBook Pro with Retina display already has an HDMI port on the right side of the laptop- so just plug in the standard HDMI cable. The problem is that I am only getting 1200 by 800 resolution.
